The story starts with the Hong-do and Yun-bok siblings ordering their “iced hot cappuccino” coffee at Yulje Hospital’s Angel-in-us coffee shop and dropping by at Jae-Hak and Seon-bin, who are sitting at the coffee shop sharing their most embarrassing internship moments. Jae-Hak compliments both of them wearing white coats, confirms Hong-do joining cardiothoracic surgery department and builds up his expectations on Yun-bok too, but she politely says she wants to join neurosurgery department instead. Seon-bin jumps in to give her a treat from her department and leaves together with the siblings. Gyeo-ul, also wearing her coat, joins him. Jae-Hak disappointedly says that no interns will be joining CS department this year.
In the morning, Jun-Wan catches the banana Jeong-Won throws at him for his quick breakfast before leaving the apartment. After Jeong-Won mentions that Gyeo-ul’s brother will be moving in to his fiancé’s place, Jun-Wan curiously asks him when does he plan to get married. Jeong-Won says he plans to propose to Gyeo-Ul soon. This made Jun-Wan feeling saddened being alone and asks if he could live together with them. Jeong-won was disgusted by the idea, takes back the banana and calls him a “freak.”
Seok-min, sweetly together with Seon-bin, joins Jae-Hak and Gyeo-ul back at the coffee shop. Seok-min, who previously left Yulje hospital for a year, just joined back after realizing that he needs to learn a lot from Song-Hwa as a doctor and surgeon. By becoming a skilled doctor and surgeon, he can earn more money. A disappointed Seon-bin was not expecting that money was the driving force behind Seok-min coming back to Yulje, but Seok-min continues saying he wants to make his parents live a comfortable life, buy a car and a house for himself and to get married too. Jae-Hak and Gyeo-ul quickly picks up they are witnessing a marriage proposal. In response, Seon-bin tells him that she has money (which is an indirect ‘yes’ to his marriage proposal) and he does not need a dime to spend. The scene ends with the couple leaving for a walk and bells ringing as a background song.
Song-Hwa and Ik-Jun catches up at Yulje Hospital’s Angel-in-us coffee shop for thirty minutes before his conference. They sat down with their coffee and she tells Ik-jun her schedule – has regular surgery advises, will be giving three lectures this month as part of Brain Tumor Committee and will be doing two surgery inquiries in the next two weeks for the Daddy-Long-Legs sponsorship. Yet, after a quick request from Dr. Chun she says “yes” without hesitation. Ik-Jun tries to teach her how to turn down requests but Song-Hwa tells him that she can handle this much.
Min-Ha is instructing Hong-do a simple task for the surgery later, just waiting and doing the procedure once the professor says the cue “Richardson, out”, he will get A+ for his grade. Hong-do nods as Min-Ha confirms if he understands, even if he looks unsure. During the actual delivery, Seok-Hyeong calls for “Richardson, out” and Hong-do went straight out of the delivery room while holding the richardson retractor with both hands – completely puzzling both Seok-Hyeong and Min-Ha. Seok-Hyeong questions Min-Ha if she properly taught him and Min-Ha says yes and will train him again. She then gives a glare at Hong-do’s back for not doing what is expected of him.
A patient comes into the emergency with critical condition, unfortunately, the guardians will not arrive for at least an hour according to Seong-Yeong, who is an intern from the same hometown with the patient. Sung-Young asks where did the boyfriend of the patient go and Dr. Bong sighs saying he went home after the hospital treated his minor wounds. It has been known that both boyfriend and girlfriend rode the motorcycle but only the boyfriend was wearing the helmet. He sped, violated the signal and crashed on a speed bump.
Song-Hwa operates on the patients and struggles to stop the bleeding. Although the surgery is a success, she later tells the patient’s guardians that their daughter’s injuries were severe and there is no guarantee that she will regain consciousness.
Seong-Yeong stays by the patient’s side after the surgery as he feels a kinship because they both came from the same town. He talks to her until the patient slightly nods as if agreeing to him. After a clear response of moving her left hand as he instructed, he quickly calls out to Song-Hwa to check on her.
Yong-bok checks a patient’s tube and inserts a new one without removing the old tube. Gyeo-Ul visits the patient after Yong-bok returns to the nurse station and seeing the patient surprises her. She quickly apologizes on behalf of Yong-bok and the patient assures that he completely understands and not to scold her.
Gyeo-Ul operates on a patient but struggles to stop the bleeding. He calls for Ik-Jun who arrives at the operating room at that exact moment. After the surgery, Ik-jun invites Gyeo-Ul for a coffee at his office. Ik-jun tries to cheer her up because Gyeo-Ul is upset that she has not improving at all and still make a lot of mistakes. He tells her he has assisted 500 liver surgeries before becoming a lead surgeon himself. Experience just takes time.
Ik-jun tells Gyeo-Ul that Jeong-Won made a hilarious mistake while he was an intern by giving the doctor a drawer when the doctor asked for a “seolapja.”But it was actually Ik-jun, not Jeong-Won. While the band is eating chicken at Seok-Hyeong’s house, Jeong-Won calls Gyeo-Ul and defends himself over the phone but Gyeo-Ul seems not to be convinced.
Jun-Wan and Ik-jun recall and laughs at Song-Hwa’s rookie mistake that while doing patient positions, she misheard her professor says “the wall” instead of “the pillow first”, so she stood against the wall.
When Seok-Yeong was first-year resident, his patient’s illness got worse and he has to pronounce death for the first time. The patient was hist first patient and was an expectant mother who died due to ammonitic fluid embolism. He tried for four times but ended up bawling at the floor with the husband. The nurse ended up calling the professor who then pronounce the death. The professor scolded him for embarrassing all his fellows since he has been taught to never show emotions in front of their patients.
Seok-Yeong turns to Jun-Wan, saying that he is a coldhearted jerk and probably does not have any memories like that. Ik-jun says that he has experienced it too during his first-year residency. He says Jun-wan kept his composure in front of the patient’s family but bawled his eyes out in the restroom. He regretted not showing his emotions and could have cried in front of the family.
Now that they are the professors, they all agree in silence that their new resident doctors can show their emotions but remember to not cross the line.
Yong-bok sees her patient off as he was discharged and is thanked for. Hong-do is seen sleeping on his desk with a note to himself “Richardson, out… Get things together” while someone comes in and leaves a cup of noodles with the note “I made worse mistakes when I was an intern, cheer up, from chuchu.”
